With the Healthcare Bolt-On on, every clinical check-in comes with an intake form. Patients fill it in on their own phone or your front-desk tablet while they wait.
What the patient sees
- Under Clinical visit, they pick New Patient or Returning Patient. Nothing guesses this for them.
- They choose a service and fill in the intake.
- The body map asks Where are you having a problem today?. They tap an area, rate the Intensity today from 0 to 10, and say what makes it worse and better.
- They read the consent, tick to acknowledge it, and sign by typing their full name. A patient under 18 also gets parent or guardian fields.
Returning Patient stays greyed out until the patient is verified and has a completed visit at this exact location. A name alone never unlocks it, so nobody can pull up someone else’s history.
Opening the intake on another browser
The intake opens on the phone that checked in, or on your tablet. If the patient opens it anywhere else, for example inside their email app, they tap Send me a code and enter the 6-digit code sent to their check-in email. If you have no email for them, they enter their date of birth instead.
Ready to call
A patient whose intake isn’t done shows Intake pending · not ready to call on My Day, so nobody calls them too early. When they finish, they’re ready.
When a patient needs help
On My Day, under the patient’s intake:
- Assist with intake to help them through it.
- Override intake if they can’t finish it. Pick a reason (unable to complete the form, technical difficulty, the provider will collect it verbally, or other), then tap Waive intake & continue. The reason is kept on the record, and a waived intake is never marked as completed.
Your own intake wording
Your intake starts from a standard chiropractic form. Before seeing real patients, fill in every part marked [FILL IN: …] with your clinic’s own details.
