How to write and sign SOAP notes on My Day

Each visit gets a SOAP note right on My Day. The patient’s intake answers are already in it, so you don’t type the same problems twice.

Write the note

  1. Open the note from the patient’s visit on My Day.
  2. Work through S · Subjective, O · Objective, A · Assessment and P · Plan.
  3. Tap Save & continue to save each part and move on. Nothing you type is lost if a save fails; you can try again.

Copy from a past note

Under Copy forward from prior finalized encounter, pick the parts you want and tap Copy selected sections. Only your own Objective, Assessment and Plan can be copied. The patient’s Subjective is never copied forward, and copied text keeps the date it came from.

Sign it

On Review & Sign, tick the attestation box and tap Sign. Signing records your review, your attestation and your signature together, then locks the note. Only the provider can sign, and a signed note can’t be changed.

Finish the visit now, the note later

Tap Finish service & defer documentation. You can take the next patient right away. The room frees up, and the visit waits for its note.

To finish it, tap Resume documentation. When nobody is waiting, My Day shows Finish your notes with your unfinished notes, oldest first, each with a Finish note button.

Your clinic can set how many unfinished notes a provider can have before My Day reminds them, in Unfinished Notes. Going over the limit never stops you taking the next patient.
